Simple Screen: With clinical observations and one page written results and recommendations: 150.00

Consultation: On-Site or In-Clinic; records review with written comments upon request; representation; advocacy; professional witness; environmental assessment; or service per indivdual request:  150.00 / HOUR

Therapeutic Treatment: Includes individualized treatment plan; re-assessment; and discharge summary with table/graph of progress:  125.00 / HOUR.

Modalities: Heat/cold, splinting, et cetera: as quoted.

Mileage Outiside Whatcom and Island Counties:  mileage as quoted.

Video Documentation, one VHS tape of a single session:  25.00 / PER TAPE.
